Charleston Dental Studio (CDS) provides every patient with a copy of our current Notice of Privacy Practices (NPP), as required by HIPAA (45 C.F.R. § 164.520), outlining how we use and protect your health information and your rights under HIPAA.
Notice of Privacy Practices (NPP) Policy
NPP Distribution & Acknowledgement
- A copy of the NPP and an Acknowledgement of Receipt form will be provided to all new patients on or before their first in-office appointment.
- Patients receiving care via telehealth, phone, or email will receive the NPP and Acknowledgement via email at the time of initial contact.
- If the Acknowledgement form is not returned electronically, it will be completed at the first in-person visit.
- If a patient refuses to sign, this will be noted in the record.
- In the event of an email failure, a hard copy will be mailed.
- For emergency visits, the NPP will be provided as soon as possible and the emergency noted in the chart.
Availability
- The NPP is posted in the front office and available upon request (printed or emailed).
- The current version is prominently posted on our website, within two clicks of the homepage, and also available as a downloadable PDF.
- If updated, the revised NPP will replace the previous version online and be available in print or email by request.
- All previous NPP versions are kept on file for a minimum of 6 years.
